The white dots in the foto are small cracks in the mylar screen. Behind the mylar screen are the MLCs (Multi Leaf Collimators). Varian medical linear accelerators (LINACs), used to deliver radiation to cancer patients, have 120 leaves in the MLC system.
This is a Varian LINAC. See the note to see what 1 leaf is. Each leaf in a Varian LINAC is made of tungsten.
The leaves move independently of each other. This allows the radiation beam to focus more radiation dose on the patient's tumor, while sparing adjacent organs which might be sensitive to radiation.
